Your sample opt-in form must contain the following elements:

Requirements:

1. Your logo
2. First name field
3. Phone number field
4. Subscription Consent check box and disclaimer “By entering your phone number and checking this box you are subscribing to receive mobile alerts from [campaign name].“
   Note: Your checkbox must be unchecked by default
5. Messaging disclaimer “Message and data rates may apply. Frequency varies. Text HELP for info. Text STOP to [your short code] to cancel.”‍
